Social Workers based across the Bradford District support people in their own homes and community by assessing a person’s needs and if needed, organising a package of care or referring to Bradford Council assistive technology teams.

Hospital Discharge
Social Work referral to Bradford Council in-house residential homes
Short term placement upto 6 weeks with a team of experienced staff including occupational therapists who will complete an assessment, including Assistive Technology, to help maximise ability to return home.
Discharged home alongside a referral made to Safe and Sound
Discharged to a long term residential or care home
